At Westside Family Healthcare, the Mobile Health Nurse helps to coordinate clinical services for patients in our community.
During summer months, the Mobile Health Nurse's focus will be on migrant and seasonal farmworkers (MSAW).
During non-summer months, the Mobile Health Nurse will work with the MSAW team to develop and deliver healthcare to other populations in the community.
Duties include outreach and facilitating clinical care, ensuring smooth operation of the clinical team on the Mobile Health Unit (MHU), initial preparation of the patient, patient education, assisting the provider during patient examinations, phlebotomy services, and Direct Observation Therapy.Since opening our doors in 1988, Westside has been driven by our mission to improve the health of our communities by providing equal access to quality healthcare, regardless of ability to pay.

Westside Family Healthcare is an Equal Opportunity Employer that values diversity.RESPONSIBILITIES OF THE MOBILE HEALTH NURSEEngage in outreach initiatives as directed by the Associate Director of Migrant/Seasonal Agricultural Workers (MSAW) Program.
This includes building relationships with farm owners, managers and crew leaders in order to connect with migrant and seasonal agricultural workers with clinical services.Maintain the MHU in accordance with Joint Commission standards, including stocking and maintenance of diagnostic equipment, supplies and medications needed for patient care.
Perform and document daily completion of QC logs and maintain POCT before beginning patient careAssist providers and medical assistants with patient care, e.g., take vital signs, perform EKGs, collect specimens, irrigate ears, dress wounds, splint and tape injuries, perform selected diagnostic tests, and assist with pap smears, physical exams, and minor procedures when necessaryProactively and continuously reviews patient medical records to identify any care needs requiring interventionEducates patient/family about medical conditions (existing or newly diagnosed) to assist them in appropriate self-managementAttend community meetings and participate in community events/fairs to support the care for patient's we partner withLead team outings and outreach during nurse led health screening eventsPM22Requirements:
Minimum Objective QualificationsState of Delaware (or compact state) Registered Nurse licensure with no restrictions or disciplinary actionMinimum of 2 years' experience in the area of ambulatory care or comparableCurrent certification in Basic Life Support for Healthcare ProvidersAmbulatory Care Certification or commit to obtaining upon hireFluency in written and spoken English and spoken Spanish, including knowledge of medical terminologyBecome trained in phlebotomy within six months of hireAt least six months of experience with electronic medical record softwareAbility to travel independently to locations (reliable transportation, insured vehicle and valid driver's license)PIadcc8ad5b86c-26289-#1
