*ROLE SUMMARY*
Home Care positions travel to various locations within the city and require the use of a personal vehicle with insurance coverage. This position works under the supervision of Home Care leadership. The RN Home Care assesses, plans, implements, evaluates, and supervises patient/family care in the home based on age-specific needs/expected outcomes according to policies and procedures. Accepts responsibility for direction of coworkers in the implementation of care plans.
*CORE RESPONSIBILITIES*
- Uses the nursing process when planning and providing patient/family care.
- Coordinates patient care activities and maintains communications with other disciplines.
- Oversees the delegation of specifics of Care Plan to other employees.
- Teaches the patient/family about the patients care needs, disease prevention and health maintenance measures.
- Performs technical procedures safely according to policies and procedures.
- Maintains accurate and timely records according to policy.
- Demonstrates self-directed behavior.
- Attends a minimum of 15 hours of continuing education on a yearly basis.

* Ability to work independently with excellent organizational skills, time management, sound judgment and attention to detail
* Excellent verbal, written, and interpersonal communication skills

* Wyoming Registered Nurse (RN) License or enhanced Nurse Licensure (eNLC)
* Valid USA drivers license
* Must have clean driving record that follows Cheyenne Regional Medical Centers driving guidelines
* Cheyenne Regional AHA RQI within 14 calendar days of start date
*PREFERRED QUALIFICATIONS*
* Bachelors degree from an accredited school of nursing
* Home care nursing experience
