Work Schedule: Full-time position requiring flexible scheduling and rotation on an on-call system with evening availability.

POSITION SUMMARY Provide case management services to young adults enrolled in the Axis 180 program to include case management, housing/education/employment/life skill instruction, and ongoing assessment of participant's progress toward self-sufficiency and community-based housing.

ESSENTIAL FUNCTIONS STATEMENT(S)

Program Development A. Promote and coordinate development of program utilizing existing agency program services.
B. Develop and maintain a working relationship with community partners.
C. Develop and maintain a working relationship with community resources that will provide participant support.
Program Management A. Ensure consistent and ongoing communication with the team members, supervisor, referral sources & guardians as necessary.
B. Complete thorough documentation of participant services including intakes, assessments, individual transition plans, progress reports, and discharge reports.
C. Attend and participate in community meetings as scheduled.
Participate in multidisciplinary teams to make recommendations on applicant enrollment, clinical consultation and team meetings.
A. Attend training as requested and/or required to meet local, state and accreditation requirements and ensure best practice.
B. Participate in data collection, eligibility documentation and evaluation studies.
Participant Services A. Complete assessment of participant needs utilizing program sanctioned tools to identify and prioritize financial assistance, behavioral health services, independent living skills instruction, employment, education, chemical dependency services, and health care.
B. Develop, monitor, and implement recommendations of individual transition plan.
C. Provide regular consultation for participants as needed.
D. Assist participant in applying for food stamps, WIC, bus passes, and other assistance as appropriate.
E. Transport participants as needed to appointments and interviews.
Other A. Participate in rotating on call for participants to utilize in emergency or crisis situations.
POSITIONS QUALIFICATIONS
SKILLS & ABILITIES

Education: Bachelor degree in social work or related human services field

Experience: Minimum of two years related work experience with adolescents, knowledge of community resources and abilityto establish therapeutic relationships with youth preferred

Skills: Excellent oral and written communication skills

Certifications & Licenses: Licensure with the South Dakota Board of Social Work Examiners as a Licensed Social Worker orLicensed Social Work AssociateValid driver's license and reliable transportation Other Requirements: Ability to lift a minimum of 50 lbs PHYSICAL DEMANDS Ability to Stand, Walk, sit, manually manipulate, reach outward and above shoulders, speak, climb, crawl, squat, kneel, and bend on a daily basis
Capability to Lift, carry, push or pull up to 100 lbs on a daily basis
