Residential Case Manager- Sutherland Apartments (Part Time)
We have an excellent opportunity for new graduates! Currently seeking an energetic Residential Case Manager to assist in the delivery of psychiatric, rehabilitation, and supportive services to adults living in our supervised apartments in Woonsocket.
The Residential Case Manager will provide a safe, stable space for those clients with mental health issues to help them achieve personal goals and improve quality of life.
We are searching for an individual who is interested in the opportunity to work with clients to help them develop social and daily living skills and work to address their needs.
Provide case management services to residents in accordance with the client's individual treatment plan, goals, philosophy, and standards defined by the agency.
Continually assess and evaluate clients, providing support services, crisis intervention, medication monitoring and education on as-needed basis.
As a Case Manager you will educate clients, their families and significant others regarding mental health issues, substance abuse and the impact on their lives.
Facilitate referrals to primary health care providers and specialty care providers (health and behavioral health) in the community as well as within the agency and provide support as necessary.
This is a full-time position with benefits.
Education/ License Requirements:
Associate's Degree in Human Services is a requirement; BA/BS is preferred.

CPR/AED & First Aid required.
