Full Time, Day Shift Monday - Friday Only. No On-Call or Weekends
Location will be to various facilities and patient homes within the Harrisburg, West Shore, Hershey and York area.
Are you a compassionate healthcare professional with a heart for serving seniors? If so, we invite you to join us as a LPN Field Nurse specializing in palliative care services. This unique role offers you the opportunity to make a significant impact in the lives of senior patients facing serious illness.
You'll manage and coordinate palliative care, ensuring timely and accurate orders for testing, medications, and referrals. Communication with patients, families, and providers, along with updating treatment plans and addressing concerns, is key. Accurate documentation of health information is essential.
Providing hands-on care during patient visits, including taking vital signs and assisting providers, is crucial. You'll handle medication management, coordinate referrals, therapy orders, and telemedicine appointments, manage prior authorizations, and update care plans as needed. Effective communication and HIPAA compliance are vital.
Palliative Care LPN Qualifications:
• LPN with current licensure to practice professional nursing in Pennsylvania.Minimum one year of experience; experience in the area of palliative care, geriatric care, or hospice nursing preferred.
• Minimum of one year of experience; experience in the area of palliative care, geriatric care or hospice nursing is preferred.
• High proficiency with computers, Microsoft Office, cellular technology, and EMR required.
• Demonstrated ability to prioritize tasks and handle pressure and delicate situations and work independently.
• CPR certified and able to perform the physical tasks that are needed in this role.
• A state driver's license in good standing, and maintain an automobile in good working order.
