About the opportunity:

This position will be responsible for comprehensive case management and care coordination to ensure a consistent and coordinated delivery of services for high-risk, high-acuity mental health consumers in Eagle County. The goal of the Behavioral Health Case Manager position is to reduce barriers to care, work directly with the client to improve the mental health outcomes through supportive case management, access to services to address social determinants of health and behavioral health, early intervention, risk reduction, client education and empowerment. The Behavioral Health Case Manager will work within the context of a client-centered, recovery-oriented, wraparound approach, in partnership with mental health providers, agencies and physicians to promote timely access to needed care and services, comprehension and continuity of care, client empowerment and the enhancement of client and family wellbeing.

What you will do:

Client assessment of clinical need, level of care and associated factors that may impact care (i.e. SDoH)
Care coordination with mental health and medical providers to improve access and continuity of care (may include home visits, assistance in attending appointments or other activities that support continuity of care)
Convening care coordination meetings with providers when necessary to support the needs of the client
Care plan development and goal setting
Facilitation of referrals and coordination with community resource providers
Client (and family) skill building - teach skills and help increase client self-sufficiency
Updating and maintaining client records
Tracking client outcomes
Client advocacy
Outreach, engagement and relationship building with medical, mental health and community providers
Attendance and engagement in outreach and educational events
Role models the principles of a Just Culture and Organizational Values.
Perform other duties as assigned. Must be HIPAA compliant.

What you will need:

Experience:
At least one year of experience in a human services, behavioral health, or related field, required.
Understanding of Colorado laws and regulations as they relate to the mental health practices, privacy, confidentiality, supervision, and ethical delivery of care, preferred.
Experience with ensuring accurate and timely data collection and data reporting to include Medicaid billing or reporting requirements, preferred.
Direct experience working with diverse populations, preferred.
Experience with behavioral health specific electronic mental recording, including documentation standards, coding requirements, and minimum standards of care, preferred.
Understanding of the American Psychological Association (APA) ethical codes and applicable statutes, preferred.
Experience in HIPAA and privacy laws with specific understanding of Part 2/42 CFR restrictions.
Rural behavioral health experience with specific knowledge of resources and community agencies specific to Eagle County, preferred.
Bilingual, strongly preferred.
License(s): Current, valid driver's license and ability to pass Vail Health's Department of Motor Vehicle Report criteria required
Certification(s): N/A
Computer / Typing: Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements
for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc
Must have working knowledge of the English language, including reading, writing, and speaking English.

Education: Bachelor's in human services or related field required. Master's degree in social work, or related human services preferred.

Pay is based upon relevant education and experience per year.

Yearly Pay:

$62,429 - $93,665 USD
