About PACE

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (westhealth.org), is a new non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

One Team One Mission

Position Summary:

Under the supervision of the Center Director and the HCC Lead the CNA/Personal Care Aide provides support to the team in the delivery of personal care to participants and to participants in need of companionship services to and from medical appointments. Services include participant care activities such as showering, toileting, serving, feeding and other Activities of Daily Living. Position will assist with home delivery of meals and medical supplies. Position may also assist in other areas such as kitchen, primary care and therapy.

Essential Functions:

Provides direct personal care to participants as directed or assigned. Duties will include showering, toileting, laundry and feeding assistance and companionship services to and from medical appointments.
Assists other staff in carrying out program assignments.
Provides support and assistance with meal and snack service.
Will provide assistance in the kitchen as needed/assigned.
Will provide assistance in therapy as needed/assigned.
Responsible for medical chart daily entry of Day Center activities for Participants.
Assists in maintaining patient areas in a clean and orderly manner. (Cleans and organizes program areas including cleaning of equipment and shower/bathroom areas. Disinfecting)
Assists with home delivery of meals and medical supplies.
Assists with front desk phone coverage as needed/requested.
Follows infectious disease precautions to prevent the spread of organism.
Attends center staff meetings and in-service educational training programs.
Provides home assistance, assists with ADLs and IADLS, transfers, companionship to doctor's appointments, etc.
Responsible for completing all CNA/PCA and trainings.
Performs clerical and administrative work as required including making and answering phone calls, assisting with the implementation of I-SAT and HOS-M surveys. Entering participant data in EHR system and assisting covering reception.
Communicates effectively with participants and co-workers. Works effectively in a team setting.
Participates in interdisciplinary team meetings as needed.
Adheres to GMWP attendance and punctuality policies and practices.
Attends meetings as required and participates on committees as directed.
Performs other related duties as assigned or requested.
Work for this position is required on site Monday-Friday.
Must be flexible and open to change.
Will require use of personal vehicle.

Education and Certification:

Must possess, as a minimum, a GED or high school diploma.
CPR/BLS certification required.
Bilingual in spanish.
Clean driving record
Active driver license
Proof of auto insurance.

Skills & Experience:

Knowledge of the principles and techniques involved in relating to frail elderly persons.
Skill in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants.
Demonstrates sensitivity to the needs of older adults as evidenced by a caring approach, patience in providing care, and respons.
