Ensure the overall health and safety of the client at all times
Address immediate medical needs of clients (phone or in person)
Maintain confidentiality of client information at all times
Complete documentation and submit to RN weekly
Administer medications to clients as prescribed and documented on MAR
Complete documentation (MAR, client specific monitoring) daily
Participate in client's IDT (when necessary) and report any perceived areas of concern or accomplishments
Complete incident reports for all unusual incidents involving the client and immediately notify RN of occurrence
Represent the agency and client in a positive manner
Demonstrate understanding of agency policies and procedures
Serve as a liaison between SBH and other agencies, community members, and medical professionals
Work with physicians and specialists to complete plan of care
Assist in obtaining informed consent for medications/treatment
Assist in ensuring physician's orders are current and correct, taking off physician's orders
Facilitate and attend (when necessary for completion of plan of care) medical appointments
Routine monitoring of client specific issues (seizures, bowel habits, blood pressure, etc.)
Other client specific duties as assigned by the RN
