• Responsibilities
• Provide behavioral health counseling services.+ Conduct patient assessments and establishes therapy treatment goals.+ Administer individual and group therapy.
Maintain thorough and confidential records of observations.+ Evaluate progress and effectiveness of therapy interventions.+ Provide patient with information regarding resources and therapeutic activities in the community.+ Work directly with the patient, family, physician and community providers to ensure active and collaborate care from the initiation of treatment to the time of discharge.
• Requirements
• Licensure / Certification / Registration:
• One or more of the following required:
• Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.+ Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.+ Social Worker specializing in Clinical Practice credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.+ Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.+ Therapist specializing in Marriage & Family credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.+ BLS Provider preferred.
American Heart Association or American Red Cross accepted.+ Supervisor credentialed from the Wisconsin Department of Safety and Professional Services preferred.Education:
• High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
