Mount Alverna Village , a part of Franciscan Ministries, is a vibrant senior living community in Parma, Ohio. We are a continuing care retirement community (CCRC), offering a variety of senior lifestyle options including independent living, assisted living, assisted living memory care, and skilled nursing/long-term care. Additionally, the onsite rehabilitation clinic offers inpatient and outpatient physical, occupational, and speech therapies, and state-of-the-art programs designed to enhance the quality of life for residents and patients in the local community.
The Wellness Nurse is essentially responsible for coordinating day to day resident care in collaboration with the caregivers and nursing team in our Assisted Living Department. This position would be available to assist staff nurse and takes routine on-call for clinical concerns. Completes resident screenings, plan, coordinates and implements educational programs for resident families and staff, regular screenings/assessments for current residents and screenings for potential residents of Ancora in coordination with Director of Resident Services and the Manager of Sales and Marketing, cooridinate staff scheduling for Assisted Living employees including finding coverage for call offs and other scheduling concerns, and essentially act as a right-hand to the Director of Resident Services.
Specific Tasks/Duties:
Oversees the general wellness of the Ancora residents by conducting regular health and wellness screenings as warranted, coordinating physician visits for residents, communicates with physicians and nurse practitioners for orders or notifications of changes in condition. Works with the community staff to maintain the well-being of all Ancora residents.
Coordinate and oversee schedule for Assisted Living Associats
Take associate call offs and adjust schedule accordinly
Will assist with administering medications in the absence of a staff nurse or when needed to assist staff nurses.
Screens potential residents upon request for eligibility/appropriateness for memory-support services. Completes resident screenings and develops the Service Plan based on identified needs from the screening.
Regularly reviews and updates resident Service Plans as scheduled or as needed.
Follows-up on residents who have recently been hospitalized, utilized the emergency call system or local emergency. In collaboration with Director of Resident Services and staff nurses, provides ongoing communication with families regarding changes in condition and/or new physician orders.
Work closely with Resident Care Assistants and other staff to ensure we are meeting the needs of the residents.
Collaborates with Director of Resident Services to evaluate the day to day function of the unit and program offering suggestions/recommendations for changes when warranted.
Supports and participates in continued development and education in accordance with policies/procedures, applicable regulations and community needs as directed by the Director of Resident Services.
Follows infection control/universal precaution practices in the home environment
Assists in the orientation of new staff to meet resident needs
Assists with the orientation of new resident moving in.
Maintains resident
and staff TB records and updates.
Attends continuing education offerings and in-services to update skills and contribute toward improvement of delivery or care/services.
Attends and participates in meetings as directed and determined by Director of Resident Services.
Participate in on-call rotation with the RSD for nursing coverage in the event of scheduling needs.
Qualifications/Requirements
Bachelors Degree in Health Related Field.
Graduate from an accredited school of professional nursing with either an LPN or RN license in the state of Ohio
1 to 2 years of experience in Health Care Management.
