Responsibilities:

Conduct comprehensive whole-person clinical assessments, including gathering information from various sources and performing thorough history and physical examinations.
Identify and document undiagnosed conditions, order additional studies as needed, and support successful disability applications when appropriate.
Conduct home visits and field visits as necessary.
Make individualized recommendations and referrals for programs and care based on assessed needs.
Lead the medical component of team-based care, training and advising Guides and Community Resources Guides on clinical issues and treatments.
Support care coordination, manage care gaps, and perform medication reconciliation post-discharge.
Communicate and coordinate with individuals care providers, primarily at CMHCs, FQHCs, and hospitals.

Requirements:

Master s degree or higher.
3+ years of experience in Primary Care, Value-Based Care, or as a Valid Nurse Practitioner (NP) in the state of Virginia (VA).
Willingness to conduct home visits.
Preferred: HCC Coding experience.

Qualifications:

5+ years of clinical care experience as a Nurse Practitioner, ideally in a primary care setting.
Experience working with underserved populations with behavioral health and/or SUD conditions.
Experience developing and delivering educational programs to nurses and affiliated medical staff.
Strong care management and coordination skills.
Empathy, compassion, and approachability.
Excellent listening, communication, and interpersonal skills.
Cultural humility and knowledge of trauma-informed care and social determinants of health.
Data-driven care approach.
Familiarity with local providers.
Ability to maintain professional boundaries and engagement skills in non-traditional work conditions.
Intermediate computer skills, familiarity with Microsoft Office Suite.
Driver s license, private auto insurance, and insurability under company coverage.

Compensation:

$142,500 base salary.
10% bonus potential.
Incentive: At 36 months, a guaranteed lump sum payment of $30K plus a $10K base salary adjustment.
