Great work you'll do here:

The Physical Therapist directs physical therapy treatment toward preventing disability, relieving pain, developing, improving and/or restoring muscle function and maintaining maximum performance within the client's capabilities in accordance with the plan or care/treatment.
The PT will oversee physical therapy assistants to assure appropriate implementation of the plan of care.

Evaluates the client's level of function by applying diagnostic and prognostic muscle, nerve, joint and functional abilities tests
Evaluates the home environment for rehabilitation and makes appropriate recommendations to increase the independence of the client
Supplies physical therapy services in the home as prescribed by a physician
Administers manual therapeutic exercises; instructs, motivates and assists clients in non-manual exercises and functional activities
Administers traction, massage and treatments involving the application of physical agents
Evaluates, fits and adjusts prosthetic and orthotic devices and recommends any modifications to the physician
Participates in the development, review, and updating of the care plan
Observes, records and reports any changes in the client's condition and response to therapy and makes suggestions for changes in the plan of care to the client's physician and Case Manager
Prepares clinical records (initial evaluations - including OASIS, care plans, progress notes, discharge summaries) and submits them for the client's clinical record
Participates as a member of the interdisciplinary team in case conferences and in planning and evaluating the total treatment for the client and confers with interdisciplinary team members as needed
Advises and consults with the client/family and other agency personnel
