RN/Registered Nurse Senior Clinical Quality Consultant Compact Lics Telecommute - Remote - WFH

The WellMed Quality Clinical Programs team is here to support WellMed Medical Management by delivering patient-centered, clinically collaborative telephonic outreach.
Our focus is on helping people live healthier lives through telephonic and digital engagement strategies.
We're looking for compassionate individuals to join our interdisciplinary service delivery team comprised of Registered Nurses, Licensed Vocational Nurses, and Social Workers.Position:
Registered NurseAs a Registered Nurse on our team, you'll report to the Manager of Clinical Programs and perform telephonic, patient-centered clinical consults.
Your primary responsibilities will include conducting Care for Older Adults annual pain and functional assessments, as well as Transition of Care following hospital discharge.
You'll also assist with appointment scheduling and ensure adherence to established productivity and quality metrics.Key Responsibilities:
Adhere strictly to department metrics and protocols, handling incoming and outgoing calls with professionalism and care.Conduct patient assessments and educate patients on health conditions, screenings, and necessary appointments.Communicate effectively with providers and offices to obtain necessary medical information.Document all calls and actions thoroughly within core systems.Work a flexible schedule to support business hours, including occasional Saturdays.Required Qualifications:
Active, unrestricted Registered Nurse license.4+ years of RN experience, including managed care settings.2+ years of experience in HEDIS/Star programs and call center environments.Proficiency in data analysis and Microsoft Office applications, particularly Excel.Excellent written and verbal communication skills.Preferred Qualifications:
Undergraduate or postgraduate degree.Experience with clinical data abstraction and billing/CPT coding.Bilingual proficiency, especially in Vietnamese.Perks and Benefits:
