The Center for Urban Community Services, (CUCS), is an acclaimed human services organization and one of the nation's largest providers of housing and social services. CUCS is a leader in implementing new practices, sharing knowledge, and assisting in shaping local, state and national strategies, so that persons who are homeless, low-income, living with mental illness, or have other special needs can live successfully in the community.
The Social Worker position is within the Williamsbridge Gardens Supportive Housing Program. The Social Worker reports to the Clinical Supervisor will be responsible for working with assigned group of individual and families in maintaining permanent housing, assist them in achieving full and satisfying lives in the community, help their team and program to function well and meet their contractual obligations including achieving contracted housing placement goals, and striving as appropriate to their role, to fulfill the CUCS programs' core purpose, core values and vision statements
Essential Duties:

•Provide direct services to a caseload of a mixture of approximately 15 individuals and families, including assessments, concrete services, and other services to help maintain housing goals.
•Conduct psychosocial evaluations.
•Connect individuals and families to psychiatric services.
•Connect clients to medical treatment and substance use and misuse support when applicable.
•Facilitate weekly or biweekly therapeutic, psychoeducational and supportive Groups.
•Document weekly Progress Notes in AWARDS System
•Obtain identification, public records, benefits and other applicable income resources.
•Meet with individuals and families on a weekly, biweekly and months basis, based on client stability.
•Conduct crisis intervention.
•Advocate for client with public and private institutions, which may include escorting to required or needed appointments.
•Upon Training, provide assistance with self-administering of medication consistent CUCS policies and procedures.
•Demonstrated ability to provide services to a specialized population, computer literacy and good written and verbal communication skills are required.
•Must be able to work effectively as part of a multidisciplinary team.
•Perform other related duties as assigned.
Requirements

Educational Requirements:
MSW REQUIRED - LMSW PREFERRED
•Social Worker 1:
Requires MSW, new graduate and/or 1yrs experience.
•Social Worker 2:
Requires MSW and/or LMSW with 3 or more years' experience.
•CUCS requires that all full-time social workers who provide services within the LMSW scope of practice to be licensed within 6 months from date of hire.
•Related experience in the mental health field, and/or working with HIV positive or homeless individuals is strongly preferred.
•Excellent outreach, assessment, written and verbal communication skills.
•Computer literacy required.
•Bilingual Spanish/English preferred
•Recent and upcoming graduates and experienced clinicians encouraged to apply.
All CUCS positions has a 6-month probationary period.
