A home care organization in North Carolina is seeking Nurse Practitioners and Physician Assistants to join their team!
About the Opportunity:
Nurse Practitioner Physician Assistant
Schedule: M-F 8am-5pm
Location: North Carolina - Fayetteville, Raleigh, Durham, Winston, Salem, Charlotte
Patient load: 6-8 on average
Responsibilities:
- Provide compassionate, holistic patient care, addressing diverse needs and collaborating with multidisciplinary teams.
• Deliver exceptional clinical care in-person and virtually, with in-person settings including private homes, congregate living facilities, and/or other home settings.
• Integrate practices and processes to achieve company goals.
• Maintain accurate medical records and participate in initiatives for patient engagement.
• Collaborate with external providers and partners.
