We are in search of a *RN Clinical Care Coordinator* to supervise, provide and coordinate nursing care in compliance with facility and regulatory policies and procedures and to assess residents' care needs.
• Primary functions:* Interview residents, residents' representatives and others with information related to the Minimum Data Set (MDS) assessment.
• Collect all data necessary to complete MDS forms and related documentation.
• Make appropriate referrals to other health care professionals.
• Participate and/or conduct interdisciplinary meetings.
• Supervise and participate in the overall care of the residents
• Oversee clinical coordination, review labs, assist the physician with rounds
• Participate in the facilities Quality Assurance process to review facility systems as directed
• Wage Information: * $5,000 SIGN ON BONUS
• $37.15/hour -$41.65/hour based on experience ad an RN & LPN
• \*Including $0.85/hour MI pass through pay until 09.30.24
