Provide home-based care to patients with illnesses, injuries and chronic diseases-often after discharge from a hospital, rehab or nursing facilityWork with a team of physicians and oversee LPNs, CNAs and aides providing patient careExamine patients, do assessments, document progress and report changes to their physicianEducate patients on their plan of care, prescribed medication, therapy, diet and exerciseCoach family members on the patient's plan of care, medication and home safety Ensure goals are met and coordinate discharge from servicesA few must-haves for Home Health Registered Nurses:
