At 315 Health, we are dedicated to providing comprehensive healthcare services to our patients. We are currently seeking a highly skilled Registered Nurse (RN) to join our team as an Annual Wellness Visit (AWV) Nurse. As an AWV Nurse, you will play a vital role in helping our patients achieve and maintain their optimal health through preventive healthcare services. Responsibilities:
- Conduct virtual Health Risk Assessments before Annual Wellness Visits for Medicare patients
- Evaluate patients' health status and medical history
- Perform various assessments and screenings, including cognitive evaluations, depression screenings, and falls risk assessments
- Document and identify patient needs and identify areas for patient growth and improvement
- Manage and update patients' health records and documentation
- Make referrals to community support and resources
- Evaluate for and educate on Continuous Care Program eligibility
- Collaborate with healthcare teams to ensure coordinated and comprehensive care
- Stay up-to-date with the latest guidelines and regulations regarding Annual Wellness Visits
Requirements
- Active and unrestricted Georgia RN license (compact preferred, but do not have to be a GA resident)
- Minimum of 2 years' experience as a Registered Nurse
- Strong clinical assessment and critical thinking skills
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and as part of a healthcare team
- Proficient computer skills and experience with electronic health records
- Attention to detail and accuracy in documentation
- Availability Monday - Friday during normal business hours
