• As part of your role, your duties will include:
• Carry out assessments (including risk assessments) along side the Rapid intervention team, Psychology and other professionals as required, and develop individual service designs, ensuring the Customer and/or their circle of support is at the centre of this process.
• To support your team to work alongside other providers to assess and identify support needs of individuals in crisis to prevent hospital admission or placement breakdown where possible
• Maintaining an environment that is appropriate to the needs of those we support.

• In order to be considered for the role, you will need to:
• Be passionate, possess good communication, and organisational skills.
• Be able to work flexibly across varying shifts, including weekends.
• Experience of managing staff teams
• Be able to engage, listen and communicate in a variety of ways.
• You will also need to understand the complexity of working with challenging behaviours, whilst demonstrating resilience and adaptability.
