We are looking for a highly skilled Registered Nurses
- Formal education as a Registered Nurse or Health care aide or equivalent experience (RN, LVN, LPN).
- Strong verbal and communication skills in English.
- In-depth understanding of disease processes related to vital sign monitoring (hypertension, heart failure, CKD, asthma/COPD and obesity).
- Time management and prioritization skills.
- Assessment and critical thinking skills.
- Clinical experience in inpatient settings is preferred.
- Experience in adult care or pediatrics.
Tasks:
- Focuses in conducting remote physiologic monitoring treatment management services, clinical staff/physician/other qualified health care professional time in a calendar month requiring interactive communication with the patient/caregiver during the month (99457)
- Call patients for verbal assessment and health teaching.
- Manages computer/on-line guidelines for the purpose of responding to customer queries.
- Coordinating patient care and collaborating with other members of the healthcare team through electronic medical records
- Monitor vitals and determine whether they are in or out of range.
- Assess vitals and prioritize based on patient needs.
- Maintaining patient confidentiality and respect for patient rights in an online environment
- Continuously monitoring and evaluating patient progress and responding to alert cases through remote monitoring tools.
- Answer patient consult calls etc.
Expected from you:
- Be available, accessible, and accountable to work during established work schedules and shifts.
- Follow support processes and procedures.
- Comply with organizational security and compliance standards, training, performance management, recurring management routines, manual and automated remote monitoring policies, etc.
- Must have an activeCompact License
- Must have an activeUS Nursing License
Pay Rate: $25.00 / HR
