This position will work with consumers living in independent Supportive Housing community settings.
These consumers will be enrolled in the Housing Support Program, Georgia Housing Voucher Program (GHVP) within Region 4 which encompasses 24 counties.
Under the programmatic supervision of the Residential Director, will work to ensure all regulatory requirements for the Housing Support Program are met, work closely with DBHDD, Regional Field Office, stakeholders to ensure positive relationships with region providers.
Provides evaluation and therapeutic services to Adult consumers with mental health, substance use disorders, co-occurring disorders as well as medical comorbidities.
Responsibilities include completion of complex psychosocial assessments, treatment team participation, treatment planning/monitoring and discharge planning.
May provide individual, family, and group counseling to consumers.
Must be familiar with all populations within the community setting.
Supervision of clinical services will be sought from the Legacy Clinical Director.
Individuals in this position must be able to collaborate with community referral sources and physicians to ensure continuity of care and will play an instrumental role in the success of the Housing Support Program.
Qualifications:
Must hold a clinical license in Georgia (LCSW, LMSW, LMFT, LAMFT, LPC, LAPC) and have at least 2 years experience working in the social services field; Planning, communicating, teamwork, networking, writing, listening, motivating, and critical thinking skills are necessary for success.
Requires basic computer skills.
Thorough understanding and working knowledge of all aspects of EMR.
Utilizes discretion in handling the organizations confidential information and consumer-protected information.
Must have a valid Georgia drivers license.
Must be familiar with all populations within the residential setting including, Adult MH and Adult SUD.
In order to be successful, Licensed Therapists must have knowledge of payor sources, billing requirements, and regulatory requirements.
Working Conditions Must be physically and emotionally fit to work in a stress-filled environment to meet deadlines and deal with routine and crisis situations.
May have extended periods of sitting, walking, and standing.
Requires varying hours of duty to provide monitoring, educational and/or crisis handling, as well as travel to off campus locations in Legacy or personal vehicle.

Employees in this position may be exposed to potential health hazards in the work environment.
Incumbents may be required to work extended periods of time at computer terminals.
They may experience traumatic situations, including psychiatric and /or deceased patients.
