Solaris HealthCare is looking for an Assistant Social Worker to join our team!

Assistant Social Worker Responsibilities:

Provide emotional support, counseling, andguidance to residents/patients and their families to address psychosocial needsand concerns under the direction of a Masters Level Social Worker
Assist in assessing residents'/patients' socialand emotional needs and developing individualized care plans in collaborationwith the interdisciplinary team
Maintain accurate and up-to-date documentationof social work assessments, interventions, and progress notes in accordancewith regulatory standards
Assist residents/patients and their families inaccessing community resources, support services, and financial assistanceprograms as needed
Advocate for residents'/patients' rights andinterests and collaborate with healthcare teams to ensure their psychosocialneeds are addressed effectively
Provide education and support to families oncoping strategies, communication skills, and caregiving responsibilities
Assist in managing crisis situations andproviding emotional support and crisis intervention services toresidents/patients and families during challenging times
Collaborate with healthcare professionals,including nurses, physicians, therapists, and administrators, to coordinatecare and address psychosocial issues impacting residents'/patients' well-being
Participate in ongoing training and professionaldevelopment opportunities to enhance knowledge and skills in social workpractice and geriatric care
Assistant Social Worker Qualifications:
Bachelors Degree in social work or related field
Previous experience in a social work or relatedrole preferred but not required
Strong communication and interpersonal skills,with the ability to build rapport and establish trust with residents/patientsand families
Compassion, empathy, and a patient-centeredapproach to care
Knowledge of social work principles, ethics, andpractices
Ability to work independently and as part of ateam in a fast-paced environment
Proficiency in computer applications andelectronic health records systems.
Commitment to maintaining confidentiality andadhering to professional ethics and standards
