Position Purpose: Assist in activities related to the medical and psychosocial aspects of utilization and coordinated care.
Description: The Program Coordinator I plays a crucial role in facilitating medical and psychosocial care coordination for members. This includes managing outreach calls, appointment scheduling, linking members to community resources, and addressing social determinants of health. The role involves direct interaction with healthcare providers, members, and community organizations to ensure effective care delivery and member compliance.
Day-to-Day Responsibilities:
- Conduct member outreach calls to promote compliance and coordinate care.
- Assist with appointment scheduling and follow-up to verify attendance and obtain necessary medical information.
- Connect members with community resources based on their needs.
- Monitor and assess inpatient census to support care coordination efforts.
- Screen members for eligibility and benefits, referring those without a primary care provider to Member Services.
- Coordinate transition of care activities, including communication with attending physicians and other healthcare providers.
- Data entry of assessments and authorizations into the system.
- Attend marketing and outreach meetings to represent the health plan and promote services.
- Prepare and distribute routine care management letters and educational materials to members.
Required Background:
- High school diploma or equivalent.
- 2-3 years of experience in a managed care setting, medical office, or healthcare facility with demonstrated medical administration duties.
- Thorough knowledge of customer service, utilization review, or claims processing practices in a managed care environment.
- Proficiency in operating office equipment including personal computers.
- Knowledge of medical terminology.
- Background in a medical field.
- Experience as a customer service representative.
- Familiarity with managed care environments.
