Clinical Case Manager RN/Registered Nurse - Short Term Restorative Care

• Part-time (0.8FTE) ongoing position
• Work from our Port Macquarie office + within the surrounding Hunter and Coffs Harbour area

What you'll be doing
As a Clinical Case Manager, you will play a crucial role in ensuring the delivery of high-quality clinical care to clients participating in the Short-Term Restorative Care (STRC) program. The STRC program is an innovative and flexible care initiative that provides early intervention to optimise the functioning and independence of older people, aiming to reverse and/or slow functional decline.

Key Responsibilities:

• Coordinate Wellness Services:
  Oversee wellness, reablement, and social support services based on assessed needs, including community services, multidisciplinary meetings, and administrative tasks.
• Develop and Monitor Care Plans:
  Conduct initial and ongoing comprehensive assessments, create care plans, and support customers by monitoring health and clinical needs. Provide guidance and liaise with other agencies and healthcare professionals.
• Manage Service Delivery:
  Ensure the delivery of services aligns with the customer's individualised plan and budget and adhere to program guidelines.
• Support and Review:
  Collaborate with customers and their support networks to regularly review and adjust care plans to maintain independence and meet program guidelines.
• Clinical Governance:
  Contribute to clinical governance within the STRC program, ensuring expenditures align with customer budgets.
• Professional Development:
  Commit to ongoing professional development, effective communication, and collaborative working relationships.

Who we're looking for
We are seeking an experienced Case Manager with a background in Nursing, or a related field. If you thrive in dynamic environments, excel at building positive relationships, and have a knack for IT systems, then you might be the perfect person for our team!

The ideal candidate for the role will have:

• Clinical Experience:
  Minimum of 2 years' experience in clinical care within home, community, or aged care settings; ideally 12 months experience in STRC or HCP delivery.
• Wellbeing and Reablement Knowledge:
  Working knowledge of wellbeing and reablement.
• Assessment and Care Planning:
  Advanced skills in assessment and clinical care planning to support independent living.
• Case Management and Customer Service:
  Effective case management skills and excellent customer service.
• Quality Care Evaluation:
  Ability to evaluate service provision to ensure high-quality care that meets standards.
• Interpersonal Skills:
  High level of sensitivity, understanding of issues impacting older adults, and commitment to advocacy.
• Aged Care Standards Knowledge:
  Strong understanding of Aged Care Quality Standards, Open Disclosure, The Aged Care Code of Conduct, and SIRS.
• Technical Proficiency:
  Proficiency in MS Office and Client Information Systems (Carelink+ preferred).
• Flexibility:
  Willingness to work occasionally outside of business hours.
• Bachelor of Nursing or equivalent - Current Registered Nurse.
• Current state-based driver's license.
