• JOB SUMMARY* Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports
• Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments
• Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes
• Work collaboratively to monitor and address clinical quality and gaps in care
• Provide clinical and educational assistance to support project initiatives
• Demonstrate communication skills necessary to interpret age-specific responses
• Participate in performance, improvement, and quality assurance activities and related meetings
• Provide clinical and educational support to meet program goals
• Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position
• Perform other duties as may be assigned by Management
• Required Licensure* Current NY State Board Certified Nurse Practitioner Licensure
• Required Experience* 2+ years practicing experience as a Nurse Practitioner
• Computer literacy and the ability to navigate new and emerging technology
• Highly developed written and oral communication skills; strong interpersonal skills
• Ability to work in a fast-paced and changing environment with proven ability to work with community groups
